Beginning in the 2020/2021 plan year, Office Visits* for Tier 1 and Tier 2 providers will be a flat $20 copay. Tier 3 will remain at 50% coinsurance.
Tier 1 - $20 copayment then the plan pays 100% (of the balance of the negotiated charge) per visit thereafter
Tier 2 - $20 copayment then the plan pays 100% (of the balance of the negotiated charge) per visit thereafter
Tier 3 - 50%
